End-of-track report covering all 6 phases:
- Phase 1-5: completed (regression fix, 32 broad catches, 4 rethrows, cold_start_ts)
- Phase 6: 30 INTERNAL_SILENT_SWALLOW sites migrated to proper Result[T]
