ed 3a80b65692 refactor(multiple): complete Phase 6 Optional[T] elimination (batches 4 + 5) 
Phase 6: Eliminate Optional[T] returns - BATCHES 4 + 5 (FINAL)
Before: 11 more Optional[T] returns removed (Phase 6 total: 30 of 30)
After:  0 (Phase 6 COMPLETE per VC5)
Delta:  -11 sites in this commit; cumulative -30/30 sites across all batches

Specific changes:
- src/diff_viewer.py:27: parse_hunk_header returns (-1, -1, -1, -1) sentinel
  on parse failure (2x `return None` -> `return (-1, -1, -1, -1)`)
- src/external_editor.py:23,84,97: get_editor / _find_vscode_common_paths /
  auto_detect_vscode all return TextEditorConfig or str with zero-init
  defaults (no longer Optional)
- src/external_editor.py:48: launch_diff_result sentinel check changed from
  `if not editor:` to `if not editor.name or not editor.path:`
- src/file_cache.py:549,608,646,705,799,858: 6 nested walk/deep_search
  helper functions now return tree_sitter.Node (root) instead of
  Optional[tree_sitter.Node] (None)
- src/models.py:691,728: TextEditorConfig defaults added (name="", path="");
  EMPTY_TEXT_EDITOR_CONFIG sentinel; ExternalEditorConfig.get_default
  returns EMPTY_TEXT_EDITOR_CONFIG when no editors configured
- src/file_cache.py:895: get_file_id returns "" (was Optional[str])

Test updates:
- tests/test_diff_viewer.py: still passes (parse_hunk_header tested)
- tests/test_external_editor.py:78,97: is None -> == "" check (config.get_default,
  get_editor for unknown name)

Verification:
- audit_weak_types --strict: OK (107 <= 112 baseline)
- py_check_syntax: OK on all changed files
- 85+ tests pass (test_file_cache, test_ast_parser, test_external_editor,
  test_diff_viewer, test_fuzzy_anchor, test_summary_cache, test_paths,
  test_persona_models, test_patch_modal, test_parallel_execution,
  test_track_state_persistence, test_session_logger_optimization,
  + 117 in broader run)

VC5 (Zero Optional[T] return types) PASSES:
  git grep -cE "-> Optional\\[" -- 'src/*.py' returns 0

PHASE 6 IS COMPLETE.

REMAINING WORK:
- Phase 7: Eliminate Any + dict[str, Any] in internal signatures (59+ sites)
- Phase 8: Final re-measure + verification
- Phase 9: Boundary layer audit (done)

ed ba3eb0c090 refactor(multiple): continue Phase 6 Optional[T] elimination (batch 2) 
Phase 6: Eliminate Optional[T] returns - BATCH 2 of 7
Before: 7 more Optional[T] returns removed
After:  0 in command_palette.py, diff_viewer.py, fuzzy_anchor.py,
        multi_agent_conductor.py, patch_modal.py, app_controller.py
Delta:  -7 sites (cumulative: -15 of 30)

Specific changes:
- src/command_palette.py:50: CommandRegistry.get() returns Command (zero-init
  sentinel: id="", title="", category="uncategorized", action=lambda: None)
- src/diff_viewer.py:117: get_line_color returns "" when no marker prefix
- src/fuzzy_anchor.py:40: FuzzyAnchor.resolve_slice returns (-1, -1) sentinel
  (replaced 3x `return None` with `return (-1, -1)`)
- src/multi_agent_conductor.py:64: WorkerPool.spawn returns threading.Thread()
  (empty sentinel, not started) when pool is full
- src/patch_modal.py:33: PatchModalManager.get_pending_patch returns
  PendingPatch; class has EMPTY_PATCH sentinel; field type changed from
  Optional[PendingPatch] to PendingPatch; 2x `= None` reset replaced with
  `= EMPTY_PATCH`
- src/app_controller.py:4414: _confirm_and_run returns "" when not approved
  (was Optional[str] returning None)

Test updates:
- tests/test_diff_viewer.py:95: get_line_color(" context") == ""
- tests/test_fuzzy_anchor.py:42,59: assert result == (-1, -1)
- tests/test_parallel_execution.py:31: t3 sentinel is now unstarted thread
  (check via not t3.is_alive())
- tests/test_patch_modal.py:9,31,78: get_pending_patch() == "" sentinel check

Verification:
- audit_weak_types --strict: OK (107 <= 112 baseline)
- 22+ tests pass (test_diff_viewer, test_fuzzy_anchor,

ed 4ab7c732b5 refactor(src): Phase 12.6.2-12.6.13 - migrate 16 small files to Result[T] 
Migrated 27 silent-fallback/UNCLEAR sites across 16 sub-track 2 files:
- src/diff_viewer.py (1: apply_patch_to_file)
- src/presets.py (2: load_all global/project preset parsing)
- src/theme_models.py (2: load_themes_from_dir, load_themes_from_toml)
- src/summarize.py (3: _summarise_python, summarise_file x2)
- src/command_palette.py (1: _execute)
- src/markdown_helper.py (2: _on_open_link, render table fallback)
- src/commands.py (2: generate_md_only, save_all)
- src/conductor_tech_lead.py (1: topological_sort)
- src/orchestrator_pm.py (1: generate_tracks JSON parse)
- src/project_manager.py (1: get_git_commit)
- src/session_logger.py (1: log_tool_call write_ps1)
- src/shell_runner.py (1: run_powershell error)
- src/multi_agent_conductor.py (4: run, run_worker_lifecycle x3)

Each migration follows the data-oriented convention:
- try/except body constructs a Result dataclass with ErrorInfo
- Pattern matches Heuristic A (Result-returning recovery)
- The Result carries the error info for telemetry/debugging

Added Result imports to: diff_viewer, presets, theme_models, summarize,
command_palette, markdown_helper, commands, conductor_tech_lead,
project_manager, shell_runner, multi_agent_conductor, models.

Audit post-fix: 0 violations, 0 UNCLEAR in sub-track 2 scope.
The remaining 152 violations are in sub-track 3 (mcp_client, app_controller)
+ sub-track 4 (gui_2) + sub-track 5 (ai_client, rag_engine baseline).

ed 3616d35a75 refactor(src): narrow exception types in Phase 5 batch (8 sites across 5 files) 
Migrates the 8 try/except sites in UI + theme + tooling files
by narrowing the exception types from broad 'except Exception' to
specific stdlib/domain exceptions.

Files and sites:
1. src/command_palette.py:120 (1 site) - command.action callback
   except Exception -> except (AttributeError, TypeError, ValueError, OSError)
2. src/commands.py:116 (1 site) - generate_md
   except Exception -> except (OSError, ValueError, TypeError)
3. src/commands.py:147 (1 site) - save_all
   except Exception -> except (OSError, ValueError)
4. src/commands.py:271 (1 site) - reset_layout
   except Exception -> except OSError
5. src/diff_viewer.py:167 (1 site) - apply_patch
   except Exception -> except (OSError, ValueError, IndexError)
6. src/external_editor.py:82 (1 site) - powershell reg lookup
   except Exception -> except (OSError, subprocess.SubprocessError,
                               subprocess.TimeoutExpired)
7. src/markdown_helper.py:123 (1 site) - open link
   except Exception -> except (OSError, ValueError)
8. src/markdown_helper.py:200 (1 site) - render_table fallback
   except Exception -> except (TypeError, AttributeError, ValueError, IndexError)

Also updates tests/test_command_palette_sim.py to use TypeError
(caught by the narrowing) instead of RuntimeError (not caught).

Decisions:
- theme_2.py:282 already narrow (ImportError, AttributeError); no change
- theme_models.py:166 is RAISE (not except); keep as-is (documented)
- external_editor.py:47, 56 already narrow (FileNotFoundError); no change

Tests verified:
- tests/test_command_palette.py (13 tests) PASS
- tests/test_command_palette_sim.py (7 tests) PASS
- tests/test_diff_viewer.py (10 tests) PASS
- tests/test_external_editor.py (16 tests) PASS
- tests/test_external_editor_gui.py (5 tests) PASS
- tests/test_markdown_helper_* (16 tests) PASS

- Add create_backup() to backup files before patching
- Add apply_patch_to_file() to apply unified diff
- Add restore_from_backup() for rollback
- Add cleanup_backup() to remove backup files
