update vulkan generator

change procedure file generation
- group procedure types together
- sort groups by procedure names
- overload function `load_proc_addresses`
    - kept original as `custom`
    - added `global`, `instance`, and `device` variants for simpler loading
    - added `device_vtable` variant to support multiple devices
